canceled except after thirty (30) days notice in writing to be <br /> given to the Risk Manager. The copies of these policies or <br /> certificates evidencing the same shall be filed with the Risk <br /> Manager of the City after approval of the City Attorney. <br /> 5 . The issuance of this permit by the City does not create any <br /> agency on the part of the permittee, and the permittee will <br /> hold the City of Stockton harmless for any action for damages <br /> arising out of the permittee' s activities performed under this <br /> permit . <br /> 6 . Permittee, or his contractor, shall obtain an Encroachment <br /> Permit for the installation of the well (s) . <br /> 7 . Each completed well head shall be covered with a meter box or <br /> valve 'box as approved by the City of Stockton. The top of <br /> each boxshall be flush with the surrounding ground or <br /> pavement . <br /> B . The provisions of the State of California, <br /> Department of Water <br /> Resources Bulletin 74-81 entitled "Water Well Standards : State <br /> of California" shall be applicable to this permit . <br /> 9 . All drilling locations, methods, and well designs are to be <br /> approved by the California Department of Health Services and <br /> the Central Valley Regional Water Quality Control Board. <br /> 10 . Drilling operations that create loud noise shall be confined ; <br /> between the hours of 7 a .m. and 6 ,.p -m. <br /> 11 . Portable mud tanks are to be used. <br /> 12 . Disposal . of water from the well development shall be in 1 <br /> accordance with current federal regulations . <br /> i <br /> 13 . Blocking or barricading City streets shall be subject to the <br /> approval of the City Traf f is Department . Traffic cones .and <br /> warning signs shall be used to direct traffic . <br /> 14 . Upon completion of water monitoring, the well shall be <br /> requirements of the Sana.t <br /> with the re <br /> abandoned in accordance q <br /> Joaquin Local Health District . <br /> 15 The permittee shall not close any travelled lane to facilitate <br /> installation, maintenance, or sampling of the well (s) . <br /> JAMES B .
