Madison Street <br /> Stockton CA 95202 <br /> The San Joaquin County Environmental Health Department (EHD) has reviewed the file for <br /> the above referenced site and has the following comments. <br /> This site is a contaminated underground storage tank (UST) site assigned to the EHD's Local <br /> Oversight Program for oversight of assessment and clean up activities. In correspondence <br /> dated March 3, 2005 you were notified that Remediation and Groundwater Monitoring Work <br /> Plan (Work Plan) dated February 14, 2005, submitted on your behalf by Treadwell & Rollo, <br /> Inc. was approved for implementation. <br /> In April 2005 the remediation proposed in the Work Plan, excavation and removal of <br /> contaminated soil, was conducted. A report of that work has not been received by the EHD. <br /> A report of that work is to be submitted to the EHD and GeoTracker, the State Water <br /> Resources Control Board's Internet Database by October 1, 2008. Also, the proposed <br /> groundwater monitoring wells have not been installed. In correspondence dated March 31, <br /> 2008 Treadwell & Rollo requested on your behalf modification of the location of two of the <br /> proposed monitoring wells to accommodate the location of underground utilities. The EHD <br /> approves the request. Please proceed with submittal of a San Joaquin County Well Permit <br /> Application, required $89.00 permit fee and $315.00 permit inspection fee and with <br /> scheduling the approved work.
