Kirk Larson, PG, and Robert Trommer, CHG <br /> State Water Resources Control Board <br /> Division of Financial Assistance <br /> 1001 1 Street <br /> Sacramento CA 95814 CUF Claim No. 8790 <br /> Subject: 521 Cherokee Lane N, Lodi, CA <br /> Review Summary Report—Additional Work, Fifth Review <br /> San Joaquin County Environmental Health Department (EHD) has received and reviewed the <br /> Review Summary Report — Additional Work, Fifth Review dated 02 December 2014 from Kirk <br /> Larson of the State Water Quality Control Board (SWQCB) Cleanup Fund (CUF) for the above- <br /> referenced site. The CUF concurred with the EHD Low-threat Underground Storage Tank <br /> Closure Policy evaluation that the conceptual site model was inadequate as the groundwater <br /> plume was not fully assessed, but determined through a professional evaluation that 1) the <br /> secondary source had been removed, 2) the case meets Policy Criterion 2b for vapor intrusion, <br /> and 3) that the case meet Policy Criterion 3b for direct contact and exposure to outdoor air. <br /> The EHD had directed by letter dated 21 July 2014 submittal of a work plan to complete the <br /> lateral assessment of impacted groundwater; the letter also directed the responsible part to <br /> address the other three criteria that the CUF has determined to be adequately addressed. The <br /> EHD will issue a renewed directive to submit the site assessment work plan and rescind the <br /> directives regarding removal of secondary source, vapor intrusion and direct contact. The EHD <br /> will also revise the LTCP checklist and Path to Closure pages on GeoTracker to reflect the CUF <br /> evaluations.
