The Primary Emergency Contact, Henry Garcia <br /> has not worked for this facility for at least one year. This information must be current and submitted as part of the <br /> business plan. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> Four drums with a total weight estimated greater than 500 pounds (actual weight was not available) of waste oily <br /> solid were found on site that have not been reported. Any material that meets or exceeds the reportable quantity <br /> shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD <br /> for approval. <br /> The Hazardous Materials Inventory forms for the propane were not accurate. The maximum daily amount of <br /> propane was identified as 1,280 cubic feet vs. 88 gallons (3,137 cubic feet). This information must be current and <br /> submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not show loading areas, emergency shutoffs, and the actual hazardous material handling and <br /> storage areas for propane,waste oil,waste oily solid, and new oils stored inside, along the north wall. A site map <br /> shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and drains, access and exit <br /> points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and <br /> emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as"NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System(CERS) annually for 2018 and 2019. The hazardous materials inventory shall be submitted by January 15 <br /> of each calendar year and may be submitted beginning November 1 of the previous year.
