S A N aJ O A Q U I N Environmental Health Department <br /> COUNTY— <br /> Underground <br /> OUNTY Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> MAIN STREET ARCO AM PM' 1100 S MAIN ST, MANTECA March 12, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the designated <br /> operator monthly inspection report and failed to check that they were responded to appropriately. The missing <br /> alarms include: L3 Fuel alarm on 2/13/18, L5 Fuel alarm on 2/13/18, L8 Fuel alarm on 2/13/18,T1 Overfill alarm on <br /> 4/1/17,6/22/17, and 10/18/17. During the monthly inspection,the designated operator shall review the alarm history <br /> for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy <br /> of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required information on <br /> the reports. <br /> This is a repeat violation, Class II. <br /> 309 CCR 2636(f)(1)Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The L7 87-octaine submersible turbine pump sump Veeder root 208 sensor, and L3 91-octane fill sump Veeder root <br /> 323 sensor failed to activate an audible and visual alarm when tested to ensure continuous monitoring of the tank <br /> system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak. Correct immediately by having a properly licensed,trained, and <br /> certified contractor replace the failed component with a functional component(LG 113-x listed, if applicable)and <br /> obtain a permit within one business day from the EHD. If the failed component can't be replaced immediately,there <br /> is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. During inspection,the L7 87-octane <br /> STP sump 208 sensor was replaced, retested, passed, and a retrofit verification form was provided. <br /> This is a minor violation. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> The fuel filters inside dispenser 9/10 appeared to be slowly leaking.A drip of fuel was observed to be on the bottom <br /> side , indicating a leak in the primary piping. All primary containment for the UST system must be tight. <br /> Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under <br /> permit and inspection of the EHD. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the 91-octane fill sump, and 87-octane fill sump. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician <br /> removed approximately 4-gallons, and 2-gallons of liquid from the 91 fill sump, and 87 fill sump respectively. <br /> Ensure that all sumps and annular spaces are maintained free of liquid. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> FA0007464 PR0506504 SCO01 03/12/2018 <br /> EHD 23-01 Rev.10/31/2017 Page 4 of 5 Underground Storage Tank Program OIR <br /> 1868 E.
