This information was required to be submitted by February 19, 2016. <br />This information has not been received resulting in a non-compliant status for this facility. An operator that receives <br />an inspection report shall have 30 days to submit a written response that includes a statement documenting <br />corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response <br />documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report The <br />violation from January 2016 stated that secondary containment was not constructed to prevent water intrusion into <br />the 87 product submersible turbine pump sump. <br />This is a repeat violation, Class II. <br />201 <br />CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. The monitoring system <br />certification testing report from 2014 wa s not found during the inspection. A copy of the report was located and this <br />violation was corrected on site. These records shall be maintained on site for at least three years. Monitoring <br />records Include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and <br />maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the <br />field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; <br />(6) results of inventory readings and reconciliations. Maintain all maintenance and monitoring records for the last <br />three years on site. <br />This is a minor violation. <br />306 <br />CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity, <br />The 87-product 208 submersible turbine pump sump sensor was leaning over and not located to detect a leak at the <br />earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the ead lest possible <br />opportunity. The sensor was relocated to the lowest point of the sump by the service technician and placed in a <br />vertical position in the sump. Ensure that all monitoring equipment is maintained to detect a leak at the earliest <br />opportunity. <br />This is a repeat violation, Class 11. <br />315 <br />HSC 25291(e) Water In secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br />Liquid was observed in the 7-8 under dispenser containment (UDC) sump and the 87-product fill sump. If water <br />could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of <br />property. The service technician removed approximately one cup of liquid from the 7-8 UDC sump and <br />approximately one gallon of liquid from the 87-product fill sump.
