An approved copy of the response plan was not found on site. Copies of the monitoring and <br />response plans shall be retained on site at all times. Immediately locate and retain a copy on site. <br />313. The 87 spill container failed when tested. All spill containers shall have a minimum capacity of <br />five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br />contact a properly licensed, trained, and certified contractor to repair or replace the spill container under <br />permit and inspection of the EHD. <br />404. Water was found in the vent sump. Moisture was observed at the bottom of the under dispenser <br />containment (UDCs) 1/2, 3/4, 5/6, 7/8, 9/10, and 13/14. Secondary containment shall be constructed to <br />prevent any water intrusion into the system by precipitation, infiltration, or surface runoff. The liquid was <br />removed from the vent sump and placed in a hazardous waste drum on site at the time of inspection. <br />Immediately contact a properly licensed, trained, and certified contractor to address the water intrusion <br />into the vent sump and the liquid in the UDCs, under permit and inspection of the EHD if required. <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by February 21, 2014, <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation, and any supporting documents. <br />Note: A fuel spill on January 18, 2014 was reported to the EHD via CaIEMA, butwas not recorded in the <br />on site records. No one on site was familiar with a spill on that date. Any release from the primary <br />containment that does not escape the secondary containment and was not cleaned up within eight hours <br />of detection, or any release that escapes the secondary containment shall be reported to the EHD within <br />24 hours and a full report shall be submitted within five working days. The report shall include at least <br />the following information: <br />1. Owner's or operator's name and telephone number <br />2. List of types, quantities, and concentrations of hazardous substances released <br />3. Approximate date of release <br />4. Date release was discovered <br />5. Date release was stopped <br />6.
