The 87 fill sump sensor was lying on its side and not located to detect a leak at the earliest <br />opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br />opportunity. The sensor was relocated into proper positioning at the time of inspection. <br />305. The diesel fill sump sensor failed to activate an audible and visual alarm when tested to ensure <br />continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an <br />audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor <br />was replaced and retested during the inspection. A "UST Retrofit Verification with Inspector Already <br />Onsite" form has been completed and provided to the operator and contractor. <br />307. The January 8, 2013 secondary containment test results indicate a failure at the vent sump. A <br />permit application was submitted to the EHD, but was incomplete and has not been approved. All <br />secondary containment for the UST system must be tight. Immediately have a properly licensed, <br />trained, and certified contractor repair or replace the failed component under permit and inspection of <br />the EHD. <br />402. A fuel leak was found at dispensers 1/2, 3/4, and 7/8; dripping was observed under these <br />dispensers. All primary containment must be impervious to the liquid and vapor of the substance <br />contained. Immediately discontinue use of failed piping and have a properly licensed, trained, and <br />certified contractor replace or repair the failed piping under permit and inspection of the EHD. If the <br />failed component can't be replaced immediately, there is a possibility that the UST system may be red <br />tagged to prevent fuel inputs. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION T ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector: <br />Title: <br />EHD 23-02 R.Td/19/12 v Page 3 UST INSPECTION REPORT <br />
