We <br /> have found the pian to be inadequate; therefore, Ilwe are returning your personal <br /> check. The following considerations are to be addressed should you choose to remit <br /> the workplan: <br /> I <br /> 1: Submit the results of previous investigation(s) . <br /> 2: Illustrate on plot map location of onsite and nearby septic tanks, ` <br /> leachfields, and underground public utility lines '(including water, sanitary sewer 1 <br /> and storm sewer) . 'I <br /> 3: Elaborate the proposed program, includinq methodology, to define the <br /> extent of soil and groundwater contamination. Enclosed is a recommended format <br /> with which the workplans and reports should follow. ! <br /> These issues should be addressed in a revised sampling plan which must be <br /> submitted to this office for approval prior to initiating the work plan. <br /> 1 <br /> Pursuant to the San Joaquin Local Health District fee schedule Resolution No. 85- <br /> 58, a $35.00 consultation fee shall be submitted along with the completed general <br /> Envirorimerntsi Health application form for review of the above referenced <br /> environmental assessment/remediation activity, f <br /> Each additional hour over the initial one hour of time spent in review activity <br /> will be billed at the rate of $35.00 per hour. Thel, additional amount due will be <br /> billed to t1he applicant identified on the general Environmental Health application <br /> form at the completion of requested activity, or monthly, if longer than 30 days is <br /> required for completion. <br /> i <br /> The current workload volume is being managed by a 601-day backlog of chronologically <br /> t <br /> received environmental assessment/remediation reports and/or workplans.
