ATC Associates (formerly Smith Technology Corporation) is submitting.
this letter report which summarizes well destruction activities at the subject site. Prior to field
activities, well destruction -permits were obtained from the San Joaquin County Public Health
Services Environmental Health Division. All work was performed in accordance with California
Well Standards Bulletin 74-90.
On January 6, 1998, ATC Associates supervised the destruction of site monitoring wells MWiR,
MW3, MW4, MW5R2, and MW6:using pressure grouting methodologies and well MW2 by
overdrilling methodologies. All well abandonment activities were performed by V&W Drilling,
C57 license 710678.
Wells MW1R, MW3, MW4, MW5R2, and MW6.were abandoned by filling, the well casing and
filter pack with a neat cement grout utilizing a pressure grouting tool. After the grout level in the
well casing had stabilized, the well .casing for each well was drilled out to two feet below ground
surface (bgs), and well casing fragments and debris were removed from the boring. The boring
was then filled with "lamp black" dyed concrete to the ground surface to restore the site to its
original appearance.
i
Well MW2 was overdrilled to a depth of approximately 30 feet bgs using 8-inch augers. The
boring was filled with a neat cement grout through the augers as they were retracted from the
boring to two feet bgs. The boring was then filled with "lamp black.' dyed concrete to the ground
surface to restore the site to its original appearance. The soil cuttings .generated during the
overdrilling of well MW2 were spread out onsite_and will be allowed to aerate. A copy of field
notes summarizing well destruction activities is contained in Attachment 1.
w:V4331\rVorts\1ttrpt doc 1
