Attachment 1 <br /> Page 2 <br /> 3) Within 60 days of completion that the closure, ECC shall <br /> submit to DHS a certification that the tank and associated <br /> piping has been closed in accordance with the specifications <br /> in the approved closure plan. The certification must be <br /> signed by the owner or operator and by an independent <br /> registered professional engineer. The certification must <br /> include a description of sampling methodology, sample <br /> locations, analytical results (including quality control and <br /> quality assurance) , sample chain of custody logs, field <br /> measurements, waste manifests and disposal record, <br /> photographs, and additional supporting documentation as <br /> appropriate. <br /> 4) This CP assumes there is moderate soil contamination and no <br /> groundwater contamination. If, during excavation, it is <br /> discovered that soil contamination is extensive and <br /> groundwater contamination is suspected, ECC will be required <br /> to submit a Post-Closure Plan for additional sampling and <br /> remediation of the contamination. This Post-Closure Plan <br /> would also be reviewed by DHS and Public Noticed for <br /> comments prior to approval. <br /> 5) During the closure process, ECC shall collect samples and <br /> submit them to a certified laboratory for analysis according <br /> to the matrix in Figure 1 at the location shown in Figure 2 . <br /> The sample Analysis Schedule proposed in the April 26, 1989 , <br /> CP shall not be used. <br />
