Attachment 1
Page 2
3) Within 60 days of completion that the closure, ECC shall
submit to DHS a certification that the tank and associated
piping has been closed in accordance with the specifications
in the approved closure plan. The certification must be
signed by the owner or operator and by an independent
registered professional engineer. The certification must
include a description of sampling methodology, sample
locations, analytical results (including quality control and
quality assurance) , sample chain of custody logs, field
measurements, waste manifests and disposal record,
photographs, and additional supporting documentation as
appropriate.
4) This CP assumes there is moderate soil contamination and no
groundwater contamination. If, during excavation, it is
discovered that soil contamination is extensive and
groundwater contamination is suspected, ECC will be required
to submit a Post-Closure Plan for additional sampling and
remediation of the contamination. This Post-Closure Plan
would also be reviewed by DHS and Public Noticed for
comments prior to approval.
5) During the closure process, ECC shall collect samples and
submit them to a certified laboratory for analysis according
to the matrix in Figure 1 at the location shown in Figure 2 .
The sample Analysis Schedule proposed in the April 26, 1989 ,
CP shall not be used.
