Revond <br /> Everetts Auto Parts <br /> 21102 Shaws Flat Rd. <br /> Sonora, CA 95370 <br /> NOTICE OF VIOLATION, FAILURE TO SUBMHT ANNUAL REPORT FOR THE STATE <br /> GENERAL PERMIT FOR THE DISCHARGE OF STORM WATER ASSOCIATED WITH <br /> INDUSTRIAL ACTIVITIES, (WDID# 5B55S009980) <br /> On 6 March 1998, Joseph Henao and Mark Elliott from our office visited your facility with the intent of <br /> gathering information on the status of your facility's 1996-97 Annual Report, and its efforts to comply <br /> with the terms of the General Permit. As it was explained to you the reason for the visit was that you <br /> failed to submit the 1996-97 annual storm water report. You stated that you no longer dismantle cars on <br /> the property due to health reasons, that you only have 14 cars left on site and that you are in the process <br /> of hauling those vehicles out of the property and selling them. <br /> Please submit a letter to this office by 10 April 1998 stating the approximate time in which the business <br /> will be closed and all vehicles out of the property. Once you remove the vehicles, you no longer be <br /> required to be in the program, to submit any more reports or pay any more fees. However, if you don't <br /> comply with our request, or fail to get rid of the vehicles, you would continue to be part of the program <br /> and continue to be in violation and liable for fines and enforcement actions from our Board. <br /> The General Permit contains three major elements to be implemented to maintain compliance with the <br /> Permit. These are the development of a SWPPP; quarterly visual observations of storm water runoff and <br /> any non-storm water discharges; and collection of samples of site runoff during the first hour of two <br /> rainfall events during the wet season(October I-May 31). Sampling events must also be preceded by <br /> three days of dry weather. The results of the latter two requirements are to be included in your facility's <br /> Annual Report which is to be submitted to this office by July 1" of each year. <br /> If you have any questions regarding this matter please call me at (916) 255-3142, or Joseph J.
