■VZ1 INC. <br /> -�, Task 6: Problem Assessment Report <br /> l ; <br /> Once the vertical and horizontal extent of the hydrocarbon-contaminated soil and preliminary <br /> groundwater information has been delineated and the remedial feasibility testing is complete, <br /> appropriate remedial alternatives and recommendations will be presented in a Problem <br /> Assessment Report (PAR). The PAR will include all basic data elements of the investigation to <br /> include the Remediation Feasibility Study and, if prac ical, any proposed Corrective Action Plan. <br /> The PAR will be prepared in accordance with both PHSEHD and CRWQCB standards for <br /> technical reports. The PAR shall include an executive summary of all investigation activities and <br /> results, a cumulative tabulation of all soil and groundwater data, plan views of the extent of soil <br /> contamination and groundwater contamination showing isoconcentration lines, groundwater <br /> elevation maps, cross sections, boring logs and monitoring well construction diagrams, <br /> laboratory data sheets and any recommendations of further work, if warranted. Where possible, <br /> all analytical data will be posted on maps and cross sections to reduce regulatory agency review <br /> time. <br /> All reports will be signed by a California Registered Geologist or Civil or Geotechnical <br /> Engineer, as required by the CRWQCB. It is anticipated that progress meetings with PHSEHD <br /> and CRWQCB staff will occur on at least a six-month basis. <br /> Task 7: Quarterly Reporting <br /> Brief letter reports will be prepared quarterly to meet he requirements of a Quarterly Status <br /> Report(QSR) for PHSEHD which will describe the field and analytical methods used, present <br /> the results of field and laboratory analyses and contraSt the recent data set with previous data <br /> each quarter. The reports will include a site location inap, a site plan map and groundwater <br /> 08420010.001 13 <br />
