The Robert and Cleo Vieira Family Trust September 12, 2005 <br /> Authorization to Complete Environmental Drilling <br /> And Monitoring Well Installation <br /> Page 2 <br /> • ARCO/BP will protect you from any claims for personal injury or property <br /> damage caused by ARCO/BP or its contractor during the work. <br /> • If further petroleum hydrocarbon impact from ARCO/BP's current or former <br /> operations at the service station is identified beneath your property, ARCO/BP <br /> will take all action required by government authorities at ARCO/BP's <br /> expense. <br /> • Once the monitoring wells are no longer required, ARCO/BP will remove the <br /> wells and restore the surface area of your property, at which time this <br /> agreement will end. <br /> Once the well installation activities are completed, the property will need to be accessed <br /> once per quarter to collect groundwater samples from the monitoring wells. Samples will <br /> be collected in conjunction with the sampling of the existing monitoring well on your <br /> property. <br /> We appreciate your cooperation in this matter. Please sign and return both copies of the <br /> Access Agreement form to our office. We will forward both copies to ARCO/BP. An <br /> ARCO/BP representative will then sign both copies and one copy will be returned to you <br /> for your files.
