Cherokee Lodi, CA 95240 209-333-1334 One each 10,000 gallon unleaded gasoline tank One each 5,000 gallon unleaded gasoline tank One each 3,434 gallon diesel fuel The project manager, and the health and safety coordinator is Keith A. Tallia of Oil Equipment Service. The site safety officers at the job site will be Keith A. Tallia of Oil Equipment Service. The alternate site safety officer will be Robert M. Dragomanovich of Oil Equipment Service. Oil Equipment Service will have on the site (2) fire extinguishers and one (1) first-aid kit within 50' of the tank location. See attached map. No smoking will be permitted within 50' of the tank or areas where flammable vapors are present. Oil Equipment Service shall notify the local Fire Department 48 hours before excavation shall begin. USA will also be notified';be-fore excavation of job. Tanks must be empty of product before start of job. Nor Cal Oil Co. will triple rinse tanks with soap and water. Rinse will be hauled away by Nor Cal Oil Co. waste hazardous hauler. Tanks will be dry iced, 30 pounds of dry ice per 1,000 gallons, before excavating. Oil Equipment Service shall monitor tanks with a Combustible Gas Indicator (CGI) and seal all openings before tanks will be transported. At no time will anyone cut or weld the tank. Tanks will be secured on truck after extraction for transportation. Tanks and plutnbing will be hauled by S & R Rentals Inc. to Triangle Inc. of Sacramento to be properly disposed of via destruction. Oil Equipment Service will approach all tank excavations with possible cave-ins. The following rules will be followed to prevent any mishaps: 1) soil to be inspected for stability before and after tank is removed, 2) soil is to be kept at least two (2) feet from the edge of an excavation, 3) if excavation is five (5) feet or more the hole shall be tapered, braced, or other equivalent methods shall be used to prevent possible cave-ins. Precautions will be taken at all times.
