Florido <br /> October 12 , 2018 <br /> Page 4 <br /> In regard to the blue bin holding two vials of loose pills and a liquid vaccine , this bin <br /> (blue akrobin) referenced in your inspection report is a temporary storage tray used to hold <br /> pharmaceutical items awaiting evaluation. The tray is not labeled with a hazardous waste label <br /> because the items contained therein are product and have not yet been evaluated to determine <br /> whether they will be managed through CVS ' s reverse distribution vendor or will be managed as <br /> waste at the store level . Items temporarily stored in this tray are evaluated on a weekly basis by <br /> pharmacy personnel . Upon evaluation, they are either packaged for transport to CVS ' s reverse <br /> distribution vendor or are managed though the store ' s Hazardous Waste Management Program . <br /> Labeling Hazardous Waste Containers <br /> Your item #605 relates to labeling hazardous waste containers . Specifically, you <br /> observed several items unlabeled and in non-hazardous waste containers in the back storage area. <br /> Also , you observed the pharmacy hazardous waste container without the physical state indicated <br /> on the label . As background , CVS ' s hazardous waste management program requires all <br /> hazardous waste containers to be separate for Flammables , Flammable Aerosols and <br /> Combustibles ; Corrosive Acids ; Corrosive Bases ; Oxidizers ; Toxics & State Regulated Wastes ; <br /> Universal Waste ; and Pharmacy, and labeled as " Hazardous Waste" with the appropriate <br /> accumulation start date , physical state and the content applicable . To address your concern , <br /> reinforcement training was provided to appropriate store personnel to emphasize labeling <br /> requirements for all hazardous waste containers . We have received confirmation that hazardous <br /> waste containers are currently properly labeled and all items un- containerized were placed in the <br /> appropriate hazardous waste container. Attached please find photos as evidence of properly <br /> labeled hazardous waste containers . On September 7 , 2018 Ted Lukawski and on September 19 , <br /> 2018 Chris Yerzy, CVS Health Environmental Specialists , retrained store colleagues on the <br /> hazardous waste program , including the importance of properly labeling hazardous waste <br /> containers . <br /> Based on the above , we believe your concern related to Store 9261 have been effectively <br /> addressed .
