V CVSHealth
Ms . Florido
October 12 , 2018
Page 4
In regard to the blue bin holding two vials of loose pills and a liquid vaccine , this bin
(blue akrobin) referenced in your inspection report is a temporary storage tray used to hold
pharmaceutical items awaiting evaluation. The tray is not labeled with a hazardous waste label
because the items contained therein are product and have not yet been evaluated to determine
whether they will be managed through CVS ' s reverse distribution vendor or will be managed as
waste at the store level . Items temporarily stored in this tray are evaluated on a weekly basis by
pharmacy personnel . Upon evaluation, they are either packaged for transport to CVS ' s reverse
distribution vendor or are managed though the store ' s Hazardous Waste Management Program .
Labeling Hazardous Waste Containers
Your item #605 relates to labeling hazardous waste containers . Specifically, you
observed several items unlabeled and in non-hazardous waste containers in the back storage area.
Also , you observed the pharmacy hazardous waste container without the physical state indicated
on the label . As background , CVS ' s hazardous waste management program requires all
hazardous waste containers to be separate for Flammables , Flammable Aerosols and
Combustibles ; Corrosive Acids ; Corrosive Bases ; Oxidizers ; Toxics & State Regulated Wastes ;
Universal Waste ; and Pharmacy, and labeled as " Hazardous Waste" with the appropriate
accumulation start date , physical state and the content applicable . To address your concern ,
reinforcement training was provided to appropriate store personnel to emphasize labeling
requirements for all hazardous waste containers . We have received confirmation that hazardous
waste containers are currently properly labeled and all items un- containerized were placed in the
appropriate hazardous waste container. Attached please find photos as evidence of properly
labeled hazardous waste containers . On September 7 , 2018 Ted Lukawski and on September 19 ,
2018 Chris Yerzy, CVS Health Environmental Specialists , retrained store colleagues on the
hazardous waste program , including the importance of properly labeling hazardous waste
containers .
Based on the above , we believe your concern related to Store 9261 have been effectively
addressed . Should you have any additional questions or concerns, please contact Adriana
Morales of CVS Health (401 - 644 - 8333 ) .
Sincerel
Nicole Wilkinson
Director, Corporate Environmental
cc : Greg Blount, Esq . , Troutman Sanders LLP
Karlie Webb , Esq . , Troutman Sanders LLP
Adriana Morales , CVS Health
CVS pharmacy / caremark / minute clinic / specialty
