Colleagues are assigned training modules according to the scope of his or her <br /> job responsibility . Training records are maintained online as well , and should be accessible by <br /> each store employee . We have obtained a consolidated training record for Store 9261 , which is <br /> included here for your review. Please note that the in person training performed by Mr . <br /> Lukawski and Mr . Yerzy is not recorded in the consolidated records since we consider it to be <br /> supplemental of the annually required hazardous waste training . <br /> Hazardous Waste Manifests <br /> Your item # 109 relates to sending generator manifest copies to the California Department <br /> of Toxic Substances Control (DTSC) . We have confirmed that CVS ' s hazardous waste hauler, <br /> Stericycle , provides copies of generator manifests to DTSC on behalf of CVS for all hazardous <br /> waste pickups at CVS stores in California . However, out of an abundance of caution, we have <br /> requested Stericycle to resubmit manifests numbers 009156089FLE, 01068550FLE, <br /> 010685501FLE , 010788706FLE and 010788708FLE . Attached please find the referenced <br /> manifests and the resubmission tracking number . <br /> Emerfiency Equipment <br /> Your item #302 relates to maintaining the required emergency equipment. Specifically , <br /> you observed a fire extinguisher located in the back storage area not fully charged . To address <br /> your concern, we have received confirmation that the fire extinguisher located in the back <br /> storage area has been recertified . Attached please find a photo showing the charged, and certified <br /> fire extinguisher. <br /> Container Closure <br /> Your item #403 relates to keeping hazardous waste containers closed . The CVS <br /> Hazardous Waste Program has been specifically designed to ensure the proper container <br /> management, while meeting the unique challenges of collecting and storing hazardous wastes for <br /> proper disposal in a space - constrained retail setting . The heavy- duty plastic totes with sealing <br /> lids must be kept closed all times when not adding or removing waste . To address your concern <br /> reinforcement training has been provided to store personnel on proper container management to <br /> ensure hazardous waste container closure when not adding or removing waste . As we discussed <br /> above , in the Hazardous Waste Training section, the bags unprocessed and un- containerized <br /> items found in an open white cardboard box, on shelves adjacent to the hazardous waste area and <br /> in an open bin labeled "Damages" at the register front store were subsequently placed in the <br /> proper hazardous waste totes with closed lids .
