<br /> 106 CCR 66262 . 34(d ) (2) Failed to train employees on waste handling and emergency procedures .
<br /> At the time of inspection , it could not be demonstrated that employees who handle hazardous waste ( HW) were
<br /> properly trained . The store manager, Iftikar Salim , stated that he and the store's operations manager, are the
<br /> primary employees who process items that are to be placed in the on site HW totes . At the time of inspection the
<br /> following was found in a Toxic HW tote with an accumulation start date of 7/21 / 18 (see photos IMG 1487- 1488) :
<br /> - The HW label was missing the physical state (see photo IMG 1459) . Iftikar marked the physical state at the time of
<br /> inspection .
<br /> - A Loreal Elvive shampoo was found leaking in a StrongPak bag , which scanned as Toxic HW (see IMG
<br /> 1460- 1461 ) , without any absorbent or clean-up debris .
<br /> - Five empty 1 . 7 oz bottles of Pert 2 in 1 Shampoo & Conditioner, scanned as Toxic HW, were found individually
<br /> bagged without any absorbent or clean-up debris ( IMG 1466- 1468) .
<br /> - An empty Johnson' s Baby Shampoo , scanned as Toxic HW, was found bagged without any absorbent or clean-up
<br /> debris ( IMG 1469- 1470) .
<br /> - A Softsoap Pure Zen Bodywash , scanned as Toxic HW, was found found leaking inside a StrongPak bag , without
<br /> any absorbent or clean-up debris ( IMG 1471 - 1473) .
<br /> - A Softsoap Fresh Breeze hand soap , scanned as Toxic HW, was found leaking inside a StrongPak gab , without
<br /> any absorbent or clean-up debris ( IMG 1481 - 1482) .
<br /> - A partially empty bottle of Irish Spring body wash , scanned as Toxic HW, was found without any absorbent or
<br /> clean-up debris ( IMG 1483- 1485) .
<br /> - When asked who placed the leaking containers , mentioned above , in bags and into the Toxic HW tote and what
<br /> happened to the spill clean-up debris , Iftikar stated he did not know. Iftikar stated it could be possible that since the
<br /> leaking container was in a very full tote , it may have begun leaking once it was already bagged and inside the tote ,
<br /> due to the pressure from the other items . The tote did appear full at the time of inspection ( IMG 1489) .
<br /> - A bagged Digital Temple Thermometer was found in the Toxic HW tote , although the item scanned as " Item not on
<br /> file . " When asked how the thermometer ended up in the Toxic HW tote , Iftikar was unsure .
<br /> - Two Duracell batteries , scanned as Toxic HW, were found bagged inside the Toxic HW tote ( IMG 1474- 1476) .
<br /> - A Satin Care shave gel can , scanned as Flammable HW, was found bagged inside the Toxic HW tote (IMG
<br /> 1478- 1480) .
<br /> The following was found in a Toxic HW tote with an accumulation start date of 8/7/18 :
<br /> - A container of motor oil was found leaking in a StrongPak bag , without any absorbent or clean-up debris. The
<br /> motor oil was placed in a Toxic HW tote with a label that did not include motor oil as part of the contents (IMG
<br /> 1507- 1510) .
<br /> The following was found in an open white cardboard box, labeled as " Damages , " located on shelves adjacent the
<br /> HW storage area ( IMG 1522- 1523) :
<br /> - A Loreal Repair Remedy Balm , inside a StrongPak bag , scanned as Toxic HW ( IMG 1517- 1519) .
<br /> - A Loreal Colo Rista Spray, unbagged , scanned as Flammable HW (IMG 1520A521 ) .
<br /> The above items were identified as waste by Iftikar Salim .
<br /> The following was found not containerized , on or near the shelves adjacent the HW storage area ,-
<br /> -
<br /> rea :- A partially empty Arm & Hammer laundry detergent, scanned as Toxic HW ( IMG 1524- 1526 ) .
<br /> - A box of Franzia Fruity Red Sangria , scanned as Flammabe HW ( IMG 1527- 1528 , 1538) .
<br /> - A Pets 101 Electronic Pet Feeder, scanned as Universal HW ( IMG 1529- 1530 , 1538) .
<br /> - A Homedics Thera-P Kneading Neck Messager, scanned as Universal HW ( IMG 1531A532 , 1538) .
<br /> - A damaged box of Gallo Family Vineyards Chardonnay, scanned as Flammanbe HW ( IMG 1532- 1535 , 1538 ) .
<br /> - A damaged white box containing 12 bottles of alcohol , scanned as Flammable HW ( IMG 1536- 1537 , 1538) .
<br /> The above items were identified as waste by Salim Iftikar.
