the notification procedures as described in the YRC Transportation Petroleum Product Spill Reporting <br /> Procedures Guide (Appendix A). The person detecting the spill must notify their supervisor, Equipment <br /> Service Center Manager or EC. The supervisor and/or EC will notify the Chemical Help Line. <br /> As much of the following information as possible should be provided to personnel notified of a spill: <br /> • Location and source of the spill <br /> • Approximate quantity of the spill <br /> • Type of material spilled <br /> • Date and time of spill <br /> • Cause and circumstance of release <br /> • Existing or potential hazards resulting from spill <br /> • Injuries or casualties <br /> • Corrective actions taken to mitigate the spill <br /> • If an evacuation will be required <br /> • Any details to assist in determining corrective actions <br /> • Name and telephone number of spill reporter <br /> The Chemical Help Line is responsible for determining if a spill must be reported to federal, state or local <br /> agencies and making the agency notification if needed. Any spill in which the EC or Chemical Help Line <br /> are notified is also recorded on the Spill Notification Information Checklist. Copies of these spill <br /> notification records are maintained in the Manager Equipment Service's files. A copy of the spill <br /> notification form is included in Appendix A. <br /> Notification of National Response Center(NRC) <br /> Any spill which meets the criteria for a reportable spill,as defined below,must be immediately reported to <br /> the NRC. A spill that violates water quality standards,causes a sheen or film on the water surface,or causes <br /> sludge or emulsion beneath the water surface. The information to be provided to the NRC by telephone is <br /> on the Spill Notification Information Checklist(Appendix A). <br /> The individual contacting the NRC should ask if other agencies should be notified and if written <br /> confirmation of the spill is required. The name and title of the NRC employee accepting the notification <br /> and the case number should be recorded on a copy of the Spill Notification Information Checklist(Appendix <br /> A). Spills reported to the NRC will also be reported to CEPA and to the LEPC. <br /> Written Notification to EPA Regional Administrator <br /> The Chemical Helpline will be responsible for preparing spill reports for the regulatory agencies. A report <br /> will be submitted to the EPA Regional Administrator within 60 days of an oil spill, which meets one the <br /> following requirements: <br /> • A spill of 1,000 gallons or more to navigable waters <br /> • Two spills in a 12-month period of 42 gallons or more that violate water quality standards,cause a <br /> sheen or film on the water surface, or cause sludge or emulsion beneath the water surface <br /> Oil spill report information required for submittal to the EPA Regional Administrator is outlined in <br /> Appendix A.
