' River Islands at Lathrop 5044.5.001.01 <br /> Monitoring Wells MW-13 through MW-22 January 13, 2006 <br /> MONITORING WELL LOCATION STUDY Page 3 <br /> • November 5, 1999 <br /> ' — MW-13,MW-14, MW-15,MW-18, MW-19 and MW-21 were damaged. <br /> — MW-17 was not observed. <br /> • December 7, 1999 <br /> — MW-13, MW-14, MW-18, MW-19 and MW-21 were damaged. <br /> — MW-17 was not observed. <br /> The approximate locations of the subject monitoring wells are shown in the Kleinfelder 2000 report <br /> on their Plates AAA, A-IB, A-1C and A-11). Copies of these plates are attached as an <br /> ' Appendix. <br /> SITE INVESTIGATION <br /> ' Field reconnaissance of the locations of Monitoring Wells MW-13 through MW-22, as shown on <br /> the Plates A-lA, A-113, A-1C and A-11) of the Kleinfelder 2000 report, was conducted on <br /> October 20 and 21, 2005. The reconnaissance included viewing the property for physical <br /> evidence of an existing well and/or remnants of the subject monitoring wells. A summary of our <br /> observations are presented below. <br /> ' SUMMARY OF FIELD OBSERVATIONS <br /> Monitoring Comments <br /> ' Well ID <br /> MW-13 No physical evidence of existing well; presumed destroyed <br /> MW-14 No physical evidence of existing well; presumed destroyed <br /> ' MW-15 Well in moderate condition— 1'/2-inch PVC pipe incased by metal pipe <br /> MW-16 No physical evidence of existing well; presumed destroyed <br /> ' MW-17 No physical evidence of existing well; presumed destroyed (see comments below) <br /> MW-18 Well in poor condition— 1-inch PVC pipe incased by a green plastic container <br /> MW-19 No physical evidence of existing well; presumed destroyed <br /> ' MW-20 Well in moderate condition— 1'/2-inch Plastic PVC pipe without casing <br /> MW-21 No physical evidence of existing well; presumed destroyed <br /> MW-22 I No physical evidence of existing well; presumed destroyed <br /> Based on the findings of the field reconnaissance, Monitoring Wells MW-13, MW-14, MW-16, <br /> ' MW-17, MW-19, MW-21 and MW-22 could not be located and appear to have been accidentally <br /> destroyed. Following the initial reconnaissance, ENGEO Incorporated performed a limited <br /> surface investigation on December 9 and 13, 2005. Exploration activities consisted of <br /> ' excavating 12 to 24 inches below the ground surface in the vicinity of the suspected monitoring <br /> well (MW-13, MW-14, MW-16, MW-19, MW-21 and MW-22) locations. A summary of our <br /> observations are presented below, as well as a summary observation for MW-17. <br />
