' h'g KLEINFELDER <br /> M employee uw�ed wmpairy <br /> File No. 20-4099-04.E02 <br /> June 8, 2005 <br /> ' Mr.John Fio <br /> HydroFocus, Inc <br /> P.O.Box 477 <br /> ' Dixon, CA 95617-2401 <br /> Re: Abandonment of Monitoring Wells and Piezometers <br /> ' Mossdale Village, <br /> Lathrop,California <br /> Dear Mr.Fio: <br /> Kleinfelder has completed the abandonment procedures of nine (9) damaged/unserviceable/Lm- <br /> needed monitoring wells and piezometers at the Mossdale Village development site located west <br /> of Interstate 5 in Lathrop, California in accordance with the Regional Water Quality Control <br /> Board (RWQCB) requirements and Kleinfelder's letter dated December 20, 2004, titled <br /> "Monitoring Well Abandonment Procedures, Mossdale Area, Lathrop, California". The <br /> ' monitoring wells and piezometers were abandoned in accordance with the San Joaquin County <br /> Environmental Health Department (EHD) requirements per the San Joaquin County Public <br /> Health Services Environmental Health Division Well Standards. The following summarizes the <br /> ' abandonment process. <br /> An EHD well destruction permit was prepared prior to commencing field work. The nine <br /> monitoring wells were over-drilled with a drilling rig equipped with a guide bit positioned over <br /> the existing well casing. The total depth of the over-drilled boring was approximately 21-feet <br /> below the ground surface. (The constructed depth of each monitoring well/piezometer was <br /> ' approximately 20-feet). To the furthest extent possible, the PVC well casing was removed form <br /> the borehole. The boring was grouted to the surface with a neat cement mixture via tremmie pipe <br /> to the surface. The grout was pumped through the tremmie pipe to the bottom of the boring and <br /> allowed to fill the boring from the bottom-up and flow to the surface. The abandoned <br /> ' wells/piezometers were inspected by EHD representatives Mr. Mike Infuma (6/2/05) and Mr. <br /> Jeff Wong(11/16/04 and 6/8/05). <br /> The following wells/piezometers were abandoned on the dates indicated: <br /> TMW-3 11/16/04 P-5 6/2/05 <br /> ' TMW-6 6/2/05 P-6 6/2/05 <br /> LMW-1 6/2/05 LMW-4 6/8/05 <br /> P-2 6/2/05 P-1 6/8/05 <br /> ' P-4 6/2/05 <br /> ' C20-4099-04.E021STOS L204 Page I oft <br /> ®2004 Kleinfelder,[nc.
