Based on the submitted work scope, <br /> prepared by your consultant. ATC Associates, letter dated January 29, 1999, <br /> pre-approval has been granted for the installation of three wells to 35' , <br /> sampling and reporting for a total cost of 17,633.00. This estimate will be <br /> placed in your file. <br /> You chould note that the Reimbursement Request (RR) instructions do not allow <br /> for a "lump eum" type invoice submittal. A breakdown by date, time, task and <br /> cost will need to be submitted for each invoice, accompanied with all <br /> subcontractor invoices. Furthermore, the Cleanup Fund would like to see a <br /> description of work done for each invoice submitted for reimbursement. when <br /> unforeseen changes or additions occur, an explanation must accompany the RR or <br /> those costs may not be reimbursed. Additionally, all work conducted must be <br /> in accordance with all applicable state and local requirements and regulations <br /> to be eligible for reimbursement.
