Our agency developed the form you <br /> attached to fulfill that reporting requirement. Each case is different depending on the information known about the <br /> discharge. In this case, the notification was made. The property owner at the time was informed and encouraged <br /> to do follow up testing of the well. There is no confirmed source of the 1,2 DCA found in the well and I'm not <br /> aware of any follow up testing performed. This is a private well and with no confirmed source, further action <br /> remains the choice of the property owner.
