Ladders shall be placed at an angle not more than 30 <br /> degrees from vertical and secured as necessary. Ladder side rails <br /> - shall extend at least three feet above the original ground surface. <br /> 4.9.5 .Confined Space End - <br /> Entry into confined spaces is not anticipated. However, if it is necessary, <br /> entry will be performed in accordance with all applicable federal and state <br /> regulations. Steps that will be followed prior to employee entry into. a <br /> confined space include: <br /> N • Employee training and indoctrination of confined space entry proce- <br /> dures prior to performing any such work. <br /> 'i Initial hazard assessment will be completed that includes atmospheric <br /> testing of the confined space for, at a minimum, oxygen. content, <br /> flammability, and toxic contaminants. <br /> • Mechanical ventilation, as appropriate, will be prc:1ded. <br /> _.
