WELL INSTALLATION WORKPLAN <br /> PERMITTING AND NOTIFICATIONS <br /> Upon approval of this workplan, obtain all necessary permits for well installations, encroachments, <br /> traffic safety control, construction of pump and treatment system compound and <br /> trenching/manifolding of treatment compound to excavation well. At least, a 48-hour notice will be <br /> given to all concern parties prior to start of any site activities. <br /> UNDERGROUND SERVICE ALERT NOTIFICATION <br /> All trenching and well installation areas will be marked in white paint and labeled for notification <br /> to Underground Service Alert(USA). Notification to USA will be initiated one week prior to any <br /> drilling/excavating activities and only after obtaining all necessary permits required to perform the <br /> necessary work. <br /> HEALTH AND SAFETY PLAN <br /> A site/job specific Health and Safety Plan will be developed for the drilling, trenching, compound <br /> construction work and will be on site for review and reference during job activities pertaining to <br /> this workplan. <br /> WELL INSTALLATIONS <br /> FOUR - INCH EXCAVATION/GROUNDWATER RECOVERY WELL <br /> N The 4 inch diameter excavation/groundwater recovery well will be drilled into the backfilled <br /> excavation through the previously installed traffic box (placed for this purpose), see Figure 3. This <br /> well will be drilled with 10 inch hollow stem augers through the pea gravel backfill three feet into <br /> native formation, approximately 33 feet below the surface. A soil sample will be obtained at the <br /> termination of the boring for analysis of Total Petroleum Hydrocarbons as gasoline, Benzene, <br /> Toluene,Ethyl Benzene, Xylenes and MtBE. The casing will be 4 inch threaded schedule 40 PVC. <br /> Slots from 33 to 27.5 will be 0.01 and from 27.5 to 7.5 feet will be 0.06 with blank casing to the <br /> surface. Number 2/12 sand will be used as the filter pack across the 0.01 inch screen. The well <br /> D will then be surged to stabilize the sand pack and if necessary more #2/12 sand will be added. The <br /> M pea gravel removed during drilling will be re-installed opposite and 6 inches above the 0.06 slot. <br /> The well will be surged to stabilize the sand and gravel pack and if necessary more gravel will be <br /> added. Two feet of hydrated bentonite will then be used above the gravel as a sanitary seal. Neat <br /> cement with less than 5% bentonite will be placed on top of the bentonite seal to the base of the <br /> traffic vault. A two inch thick cement/sand floor will be placed at the vault floor, leaving 22 inches <br /> of the casing exposed within the vault, see Figure 4 for well construction details. A locking <br /> thermos type well cap will be placed at the casing top to secure the monitor well. <br /> 4 Sanchez WORKPLAN 9-03 <br />
