As such,you have placed yourself in non-compliance and PHS-EHD has notified the State Water Resources <br /> Control Board. <br /> Pursuant to Section 2652(d)of the Underground Storage Tank Regulations(California Code of Regulations,Title <br /> 23,Subchapter 16)a quarterly monitoring report shall be submitted to the Environmental Health Division(PHS- <br /> EHD)and the Central Valley Regional Water Quality Control Board(CVRWQCB)every quarter until the <br /> investigation and cleanup of the site is deemed adequate. The report is to show the: <br /> - status of the investigation and cleanup activities including the results of all investigations completed to <br /> date; <br /> - proposed,continuing,or next phase of investigation; <br /> - method of cleanup proposed or implemented and schedules for the completion of the investigation and <br /> remediation of the site; <br /> - method and location of disposal of the released hazardous substance and any contaminated soil or <br /> groundwater; <br /> - manifest required for transport of hazardous substances; <br /> monitoring well data including groundwater elevation,pH,conductivity,temperature,gradient,and status <br /> of wells; <br /> analytical results of groundwater or soil sampling;and <br /> - site map showing the"zero-line"of contamination,if known,and changes in the above measurements over <br /> the last quarter. <br /> You are hereby directed to submit quarterly reports prior to the subsequent quarterly sampling event until <br /> directed otherwise by PHS-EHD. <br /> A quarterly report for the first quarter(January,February,March) 1996 is due at PHS-EHD by April 15, <br /> 1996. In addition to the EPA Methods 8015 mod and 8020 analyses required for the groundwater samples, you are <br /> hereby directed to include EPA Method 8260 modified GC/MS for MTBE for all three monitoring wells until <br /> further directed by PHS-EHD. <br /> A Division of San Joaquin County Health Care Services <br />
