If you <br /> conclude that the site investigative phase is complete, please submit an investigative report that <br /> includes the information typically submitted in a Problem Assessment Report(PAR). Conclusions in <br /> this report should be supported by data that indicate the soil contamination and groundwater plume <br /> have been laterally and vertically delineated. A discussion should be included that explains in detail <br /> why the site investigation is complete and warrants no further action and advises when the formal <br /> Request for Closure report will be submitted. <br /> Alternatively, if supported conclusions for closure cannot be presented,a work plan for further <br /> investigation should be submitted in a short period of time so this information can be collected quickly <br /> and you can complete your site investigation. If contamination in place on site exceeds allowable limits <br /> for closure,this work plan should include proposals for field testing methods that your consultant may <br /> be considering for remedial action. Underground Storage Tank Corrective Action Code requires that <br /> at least two remedial alternatives should be evaluated for feasibility and cost effectiveness and <br /> submitted to PHS-EHD for evaluation. <br /> UEC personnel have indicated that you would be willing to work with the Chevron site adjacent to <br /> Kwikee Foods to reach some type of agreement. PHS-EHD concurs with this suggestion and <br /> recommends that this agreement as well as a management plan and schedule for future <br /> investigation/remediation at the Kwikee Food site be included and submitted to PHS-EHD for <br /> evaluation. <br /> Whether an agreement with Chevron is reached or not,you will be required to advance your site <br /> investigation to the point where decisions can be made to either enter into the remedial phase for your <br /> site or submit a Request for Closure report Please advise PHS-EHD of any progress on the <br /> discussions with Chevron. <br /> A Division of San Joaquin Countv Health Care Services <br />
