In <br /> addition,it was understood and agreed upon that during this period,you would complete the <br /> necessary steps to secure access agreements and encroachment permits for offsite work and notify <br /> PHS-EHD of your status when the LOC was obtained. The PHS-EHD well permit application and <br /> $89 permit fee was to be submitted when the LOC was obtained. <br /> On August 18, 1999 the SWRCB-CUF issued you a Letter of Commitment. As of the date of this <br /> letter,the well permit application and fee and the status report detailing the progress in obtaining the <br /> access agreement/encroachment permits have not been received at this office and the required <br /> subsurface investigation has not been conducted. <br /> You are hereby directed to enact the.approved work plan and complete-the field work <br /> describe with'n'4 vr ete report of findings to PHS-EHD <br /> within ninvL In addition, if the soil and groundwater <br /> samples collected during this phase verify that the plumes are not delineated, an additional <br /> work plan, complete with well and encroachment permits/access agreements for offsite <br /> boring and/or well placement is to be included in the report. This is your official ninety <br /> (90) day notification to come into compliance and remain eligible for reimbursement from <br /> the SWRCB-CUF. <br /> Quarterly Monitoring Reports are still required for this site. PHS-EHD hereby directs you to <br /> continue to conduct quarterly monitoring of the wells associated with this site and submit a <br /> report of the activity to PHS-EHD prior to the next quarterly sampling event. <br /> Leaking underground storage tank sites are required to be in compliance with directives issued by <br /> PHS-EHD in order to remain eligible to receive reimbursement from the SWRCB-CUR Failure to <br /> comply with directives issued by PHS-EHD may result in your disqualification from the CUF and <br /> A Division of San Joaquin County Health Care Services <br />
