The permit applications and associated fees to begin the work <br /> proposed have not been submitted. In addition,there has not been a Quarterly Monitoring Report <br /> PMR) submitted to PHS-EHD for the Fourth Quarter-1999 or the First Quarter-2000. <br /> You are hereby directed to immediately resume quarterly monitoring and reporting for all of <br /> the wells associated with this site. QMRs are due by the 151 of the month following the <br /> quarter's end. The QMRs forthe missing quarters are to be submitted to PHS-EHD <br /> immediately. <br /> You are hereby directed to submit PHS-EHD Well Permit Application Forms and the $89 <br /> fee for each parcel or county right-of-way included in the approved work plan revision by <br /> ✓May 19,2000. Copies of any San Joaquin County encroachment permits are to be included <br /> with your submittals. The placement/installa�pn of the monitoring wells proposed in the <br /> work plan shall be completed by June 30,2000. The report of findings for these installations <br /> is to be submitted to PHS-EHD by September 4,2000. <br /> A quarterly monitoring event is to be completed for ALL monitoring wells for this site <br /> during this quarter(April,May,June 2000). The QMR for this event is to be submitted to <br /> PHS-EHD no later than July 17,2000.` <br /> Underground storage tank(UST) sites that have had an unauthorized release are required to <br /> investigate the extent of their release, monitor and report the groundwater quality of the wells <br /> installed for sampling the water, and evaluate the need for remediation after completing their <br /> investigation.
