PHS/EHD has prepared the following comments for your <br /> consideration and response. <br /> Tank Testing History <br /> PHS/EHD's records indicate that three 12,000 gallon dual walled fiberglass underground <br /> storage tanks were installed December 1984. The first precision tests which PHS/EHD has <br /> record of occurred on October 6, 1986 and the results indicated that the tanks exceeded the <br /> maximum loss for a tight tank. The tanks were retested on January 15, 1987. The retest <br /> results indicated that the tanks were tight; however, the retest was not a full system test. The <br /> tanks were next tested on December 16, 1987 and all but tank#2 (center) passed the integrity <br /> testing. A vapor line was repaired and tank#2 was retested on December 23, 1987. The retest <br /> results indicated that tank#2 was tight. Again on December 30, 1987, the tank was retested <br /> and passed. On January 5, 1989 , all three tanks and piping passed precision testing. On <br /> May 30, 1989, the leak detectors were tested and passed. On August 17, 1990, the lines were <br /> tested and passed. On October 18, 1991, the piping was tested and passed. On March 24, <br /> 1992 , the line and leak detectors were tested and passed. During 1992 continuous monitoring <br /> of the tanks was initiated. On March 19, 1993, two leak detectors failed testing; however, <br /> following replacement and retesting, the leak detectors passed. On November 4, 1993 the test <br /> of the tank#2 line failed. The retest following the replacement of an O-ring passed. No further <br /> testing records were available in PHS/EHD's file. In 1996, the annular fluid was reported <br /> leaking from tank#3. The tank was reportedly pumped and piping evacuated and plugged or <br /> capped . A temporary closure plan was submitted to PHS/EHD January 1997 and which <br /> expires on January 15, 1998 at which time the tank system must be upgraded to 1998 <br /> requirements. <br /> Unauthorized Release Reporting (UAR) History <br /> On June 3, 1991, an UAR was filed in response to a drive off. The dispenser was reportedly <br /> repaired. On February 1, 1992, a Proposition 65 Notification was filed in response to a drive <br /> off. On February 20, 1992, an UAR was filed in response to a drive off on February 19, 1992. <br /> On November 1, 1993, an UAR was filed in response to a drive off_ The gasoline reportedly <br /> traveled into the sewer. Fish and Game was notified. On November 2, 1993, a Proposition 65 <br /> Notification was filed in response to the November 1, 1993 drive off_ Approximately 100 gallons <br /> of gasoline product was discharged.
