(Orion) has prepared this Revocable Permit Application <br /> for Bridgestone/Firestone, Inc. (BFS). Following permit approval, these <br /> groundwater monitoring wells will provide additional site assessment for the <br /> subject property. This work is in response to a letter from Mr. Harlin Knoll of the <br /> San Joaquin County Public Health Service (SJCPHS) dated 30 August 1999. The <br /> scope of work includes installing three monitoring wells at the locations identified <br /> on the attached figure to delineate the nature and extent of impacted groundwater <br /> at the site <br /> As requested, Orion has attached associated fees, such as, a processing fee, <br /> financial security, a copy of the work plan and company insurance documentation. <br /> The original insurance form, naming the City as an additional insured, .will be <br /> mailed directly to the City of Stockton.
