The training program did not address the immediate notification <br /> contacts to the appropriate local emergency response personnel and to the unified program agency (spill reporting <br /> requirements). Training programs for all employees and annual training, including refresher courses, shall include <br /> safety procedures for the event of a release or threatened release of a hazardous material, including, but not limited <br /> to, familiarity with the facility's emergency response plans and procedures. These training programs may take into <br /> consideration of each employee. Immediately prepare or update the facility's training program, log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br /> information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System(CERS) annually for 2018 and 2019. The hazardous materials inventory shall be submitted by January 15 <br /> of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the <br /> CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 10 HSC 25508.1(a)-(f) Failed to submit revisions to business plan within 30 days of substantial change in operations. <br /> During the inspection two 115 cubic foot cylinders of compressed oxygen were found and the updated information <br /> has not been submitted. Within 30 days of one the following events, business shall electronically update the <br /> information submitted to the California Environmental Reporting System (CERS): <br /> -A 100 percent or more increase in the quantity of a previously disclosed material. <br /> -Any handling of a previously undisclosed hazardous materials <br /> -A change of business address, business ownership, or business name. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years.
