Designated <br />operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that <br />copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit <br />copies to the EHD. This is a Class II violation. <br />309 <br />CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br />The Bravo box float and chain in the Dispensers 1-2 and 3-4 UDC sumps failed to stop the flow of product at the <br />dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop <br />the flow of product at the dispenser when it detects a leak. The service technician adjusted the chain(s) and verified <br />functionality. This is a Class II violation. This violation has been corrected. <br />310 <br />CCR 2636(f)(1) Sensor in a turbine sump failed to activate an audible and visual alarm or stop the flow product. <br />The 87A and 91 -octane sensor failed to activate an audible and visual alarm at the panel when tested to ensure <br />continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct immediately by having a <br />properly licensed, trained, and certified contractor replace the failed component with a functional component (LG 113-x <br />listed, if applicable) and obtain a permit within one business day from the EHD. If the failed component can't be <br />replaced immediately, there is a possibility that the 87A and 91 -grade UST system may be red tagged to prevent fuel <br />inputs. This is a Class II violation. <br />323 <br />CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. <br />The 87 -octane (87A STP sump) VMI line leak detector failed to detect a leak when tested. All line leak detectors shall <br />be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested <br />it, and it passed. This is a Class II violation. This violation has been corrected on site. <br />Overall Inspection Comments: <br />THIS IS THE OFFICIAL REPORT FOR THE ROUTINE UNDERGROUND STORAGE TANK AND MONITORING <br />SYSTEM CERTIFICATION INSPECTION, AND SUPERSEDES THE PREVIOUS CHECKLIST ONLY PROVIDED <br />ON SITE. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by November 28, 2014. <br />Consent to perform the inspection, take photos and collect samples was given by Bali Singh, Tank Operator and <br />Facility Manager. <br />This facility's designated operator is Affordatest (Multiple individuals within the company ICC expiration date valid <br />to 2016). <br />The service technician was Ed Stearns (ICC expiration date: 11/27/2014), who had the following manufacturer's <br />certifications: [Franklin Fuel 12/27/2014].
