The October and November 2013 designated operator monthly inspection reports were not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD. This is a Class II violation. 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. The Bravo box float and chain in the Dispensers 1-2 and 3-4 UDC sumps failed to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted the chain(s) and verified functionality. This is a Class II violation. This violation has been corrected. 310 CCR 2636(f)(1) Sensor in a turbine sump failed to activate an audible and visual alarm or stop the flow product. The 87A and 91 -octane sensor failed to activate an audible and visual alarm at the panel when tested to ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed component with a functional component (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the failed component can't be replaced immediately, there is a possibility that the 87A and 91 -grade UST system may be red tagged to prevent fuel inputs. This is a Class II violation. 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. The 87 -octane (87A STP sump) VMI line leak detector failed to detect a leak when tested. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the leak detector, retested it, and it passed. This is a Class II violation. This violation has been corrected on site. Overall Inspection Comments: THIS IS THE OFFICIAL REPORT FOR THE ROUTINE UNDERGROUND STORAGE TANK AND MONITORING SYSTEM CERTIFICATION INSPECTION, AND SUPERSEDES THE PREVIOUS CHECKLIST ONLY PROVIDED ON SITE. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by November 28, 2014. This facility's designated operator is Affordatest (Multiple individuals within the company ICC expiration date valid to 2016).
