If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the south diesel satellite udc is maintained free of liquid. #23(c) The designated operator failed to check and note the correct test dates for the secondary containment tests on the October 2009 through December 2009 designated operator monthly inspection reports. The secondary containment test date was documented as 11-1-09 but should have been documented as 10-6-09. The designated operator failed to check and note the correct test dates for the monitoring system certification, spill bucket testing, and leak detector testing on the December 2009 designated operator monthly inspection report. These tests were documented as 10-13-09 but should have been documented as 10-15-09. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. #23(c) The April 2009 designated operator monthly inspection report was not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD by 11-27-10. #23(c) The designated operator failed to document all the alarms and attach the alarm history report for the previous month on the designated operator March and September 2010 monthly inspection reports. The missing alarms include: 9-3-10 diesel stp sensor alarm, 2-18-10 diesel stp, 87A stp, 91 stp, 87B stp, diesel/87 annular, and 91/87 annular sensor alarms. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
