The annual monitoring system certification, leak detector testing, and spill container testing was <br />performed on 10-27-10 and a test report was submitted to the EHD on 12-27-10, 30 days late. A copy of <br />the test report must be submitted within 30 days of the tests. Continue to ensure that a copy of the test <br />report is submitted to the EHD within 30 days of the tests. Repeat Violation. <br />24. Annual monitoring system certification, leak detector testing, and spill container testing were last <br />performed on 10-15-09 and was due by October 2010. Testing was performed on 10-27-10. However, <br />the technician David Winkler could not submit a current copy of his Incon level 2 certification. This facility <br />has an Incon TS 5000 monitoring panel for the UST system. David Winkler did not obtain a current Incon <br />level 2 certification until 12-13-10, one month and 16 days after performing the monitoring system <br />certification. The monitoring system certification testing that David Winkler performed cannot be <br />accepted. The required monitoring system certification testing for this facility is considered late. These <br />tests are required once every 12 months. Immediately schedule the monitoring system certification <br />testing and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be <br />charged at our current hourly rate.
