CLASS II. or MINOR -Notice to Comply) <br />THIS IS A REVISED REPORT ISSUED ON FEBRUARY 7, 2014 AND IT SUPERSEDES ALL <br />PREVIOUSLY ISSUED REPORTS. <br />ROUTINE MONITORING SYSTEM CERTIFICATION AND UST INSPECTION WERE CONDUCTED <br />ON OCTOBER 29, 2013. THE DIESEL LINE LEAK DETECTOR WAS NOT TESTED AND POSITIVE <br />SHUT DOWN COULD NOT BE VERIFIED ON OCTOBER 29, 2013. <br />THE DIESEL LINE LEAK DETECTOR INCLUDING POSITIVE SHUT DOWN WERE SUCCESSFULLY <br />TESTED ON FEBRUARY 7, 2014. THE NEXT MONITORING SYSTEM CERTIFICATION AND <br />ROUTINE UST INSPECTION ARE DUE BY OCTOBER 29, 2014.Please be aware that this follow-up <br />inspection, to verify diesel leak detector functionality and observe positive shut down and fail <br />safe, is being charged at our current hourly rate. <br />-Class 11 Violations cited on October 29, 2013: 104, 105, 106, 201, 202, 206 <br />-Class II Violation cited on February 7, 2014: 110 (Corrected on February 7, 2014) <br />104. Financial responsibility documents have not been submitted to the San Joaquin County <br />Environmental Health Department (EHD). Current financial responsibility documents are required to be <br />submitted annually. Log into California Environmental Reporting System (CERS), upload the required <br />information, and submit for review by the EHD. Immediately submit a copy of the financial responsibility <br />documents to the EHD. <br />105. UST Facility Operating Permit Application and UST Tank Information are not current in CERS. <br />Any change of information must be updated in CERS within 30 days of the changes. Immediately log <br />into CERS, update the required information, and submit for review by the EHD. Complete a separate <br />tank form for each tank. <br />105. UST Facility Operating Permit Application is lacking BOE Maintenance Fee Account Numbers or <br />BOE number is incorrect.
