Annual diesel leak detector testing and positive shut down verification for diesel turbine was last <br />performed on October 29, 2012 and was due by October 29, 2013. These tests are required once every <br />12 months. These tests were conducted on February 7, 2014 and were over three months past due. <br />Take all necessary precautions to ensure that all future testing is performed in a timely manner. Please <br />be aware that this follow-up inspection, to verify diesel leak detector functionality and observe <br />positive shut down and fail safe, is being charged at our current hourly rate. <br />106 & 202. Current approved copies of the monitoring and response plans were not found on site. The <br />monitoring and response plans must be current and approved by the EHD. Immediately log into CERS, <br />update the monitoring plan information, and submit for review by the EHD. Complete a monitoring plan <br />record for each tank. Immediately submit a copy of the response plan to the EHD for approval. <br />Alternatively, log into CERS, upload the completed form, and submit for review by the EHD. Maintain an <br />approved copy on site. <br />201. The 2010 and 2012 annual monitoring system certification and 2012 secondary containment test <br />results were not found on site. These records shall be kept on site for at least 3 years. Immediately <br />locate and maintain these documents onsite and submit copies to the EHD. <br />206. The designated operator employee training for all employees was not performed. The designated <br />operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator and maintain the list on site. A <br />copy of the designated operator employee training record for training conducted on October 29, <br />2013 was received on February 7, 2014. <br />Notes: <br />1. A line test was performed on October 29, 2013 by David Winkler on the Diesel line since the diesel <br />tank top turbine could not be started and positive shut down could not be verified.
