Financial responsibility documents have not been submitted to the San Joaquin County <br />Environmental Health Department (EHD). Current financial responsibility documents are required to be <br />submitted annually. Immediately submit a copy of the financial responsibility documents to the EHD. <br />Alternatively, log into the California Environmental Reporting System (CERS), upload the required <br />information, and submit for review by the EHD. Complete immediately. <br />105. UST Facility Operating Permit Application and UST Tank Information forms are not current in <br />CERS. Any change of information must be updated in CERS within 30 days of the changes. <br />Immediately log into CERS, update the required information, and submit for review by the EHD. <br />Complete a separate tank form for each tank. <br />105. UST Facility Operating Permit Application is lacking BOE Maintenance Fee Account Numbers or <br />BOE number is incorrect. Immediately log into CERS, update the required information, and submit for <br />review by the EHD. <br />106 & 202. Current approved copies of the monitoring and response plans were not found on site. <br />The monitoring and response plans must be current and approved by the EHD. Immediately log into <br />CERS, update the monitoring plan information, and submit for review by the EHD. Complete a <br />monitoring plan record for each tank. Immediately submit a copy of the response plan to the EHD for <br />approval.
