The 2010 and 2012 annual monitoring system certification and 2012 secondary containment test <br />results were not found on site. These records shall be kept on site for at least 3 years. Immediately <br />locate and maintain these documents on site and submit copies to the EHD by November 29, 2013. <br />206. The designated operator employee training for all employees was not performed. The designated <br />operator shall train facility employees for which he or she is responsible in the proper operation and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management <br />practices. <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring <br />plan. <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan. <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms. <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by November 29, 2013. <br />Notes <br />1. A line test was performed today by David Winkler on the Diesel line since the diesel tank top turbine <br />could not be started and positive shutdown could not be verified. David Winkler's certifications: <br />Petrotite Line Tester: 5-8-15, Tank Tester: 3-31-14. <br />2. The diesel line leak detector could not be tested today since the turbine did not start. Schedule this <br />test before November 1, 2013 or Class 2 violation #110 will apply to this facility. <br />3. Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by November 29, 2013. <br />4.
