The following is an itemized list of underground storage tank violations that have not <br /> been addressed for CHEROKEE LANE SERVICE STATION` as of June 13, 2017. <br /> Open violations from October 17, 2016 inspection <br /> Violation #318 -Leak detector failed to detect a 3.0 gph leak at 10 psi. <br /> The 87A line leak detector failed to detect a leak when tested and the 91 line leak detector had to be adjusted <br />*tbefore it passed. All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. <br /> mmediately have a properly licensed, trained, and certified contractor repair or replace the failed leak detector(LG <br /> 13-x listed, if applicable) and obtain a permit within one business day from the EHD. If the failed leak detector <br /> an't be replaced immediately, there is a possibility that the 87A UST system may be red tagged to prevent fuel <br /> puts. <br /> Open violations from October 12, 2015 inspection <br /> Violation#103 -Current financial responsibility documents not submitted. <br /> Financial responsibility documents have not been submitted to the EHD. The Letter of Chief Financial Officer that <br /> was uploaded into the California Environmental Reporting System (CERS) on 7-14-14 has a date of 7-14-14. A <br /> current Letter of Chief Financial Officer has not yet been uploaded into CERS. Current financial responsibility <br /> documents are required to be submitted annually. Immediately log into the California Environmental Reporting <br /> System (CERS) at http://cers.calepa.ca.gov/, and upload the required document(s). <br /> Violation#104 - YST Operating Permit Application for Facility and Tank information not submitted or <br /> current. <br /> The UST Tank Information for the87A, 87B, 91, and diesel USTs are not current in CERS. The secondary <br /> containment for the vent and vapor recovery piping should be marked none for each of these tank systems. The <br /> primary containment of the vapor recovery piping should be marked none for the diesel tank system. Any change of <br /> information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the <br /> required information, and submit for review by the EHD. <br /> Violation#107 - Plot plan/site map not submitted or failed to completely show where monitoring is <br /> performed. <br /> An accurate UST Monitoring Site Plan was not submitted. The site plan uploaded into CERS on 7-14-14 does not <br /> list the mechanical line leak detectors, the float and chain leak detection mechanism within the under dispenser <br /> t containment sumps, nor the Incon monitoring panel. A site plan must be submitted identifying the locations where <br /> monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. <br /> Violation#113-Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on 10-28-14 <br /> and a test report was submitted on 1-29-15, more than 30 days after the date of testing. A copy of the test report <br /> must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of <br /> -K54 test reports are performed in a timely manner. <br /> Violation#205 - Monitoring and response plans not current or approved by the EHD. <br /> The monitoring plan is not current. The 87A 12,000 gallon UST system and the diesel 10,000 gallon UST system <br /> should each list VMI as the mechanical line leak detector manufacturer and LD 99-2000 as the model. The <br /> V monitoring plan must be uploaded to the California Environmental Reporting System (CERS).
