1 <br /> 4.0 PERSONNEL TRAINING <br /> All personnel shall be currently certified under GISO 5192 Hazardous Waste Operations and <br /> Emergency Response prior to the initiation of any work on the Site or other comparable <br /> training certification All project personnel will be adequately trained to perform the work <br /> assigned The suspected site specific hazards will be discussed with the assigned employees <br /> during the pre-entry briefing at the Site prior to initiating any Site activity, and as necessary <br />' to ensure that the employees are aware of the details of the Site Safety Plan Personal <br /> Protective Equipment (PPE) will be available at the Site <br />' 5.0 PERSONAL PROTECTIVE EQUIPMENT (PPE) <br /> PPE listed will be provided to Site personnel by their respective employer <br />' PPE to be provided will include <br /> Half-face respirators with organic vapors/acid gases/particulate cartridges <br /> ' a Chemical resistant steel toed safety shoes <br /> Nitrile gloves <br /> • Safety glasses at a minimum and goggles if liquids are encountered <br /> 0 Tyvek coveralls <br /> All workers required to wear respiratory protection are required to be clean shaven at the <br /> ' Site Mustaches will be not be allowed because they interfere with obtaining a proper seal <br /> when using respiratory protection equipment All employees who may be required to wear a <br /> respirator will be fit tested with organic vapor cartridges and challenged with isoamyl acetate <br /> (banana oil) An employee will only be issued a size and model of an air purifying respirator <br /> for which a successful fit testing has been demonstrated Employees will be trained to check <br /> the fit of the respirator using positive and/or negative pressure tests after donning the air <br /> purifying respirator prior to entry into the potentially contaminated area <br /> ' 6.0 MEDICAL SURVEILLANCE <br /> ' A special medical evaluation will be provided if any employee develops signs and symptoms <br /> of overexposure to the suspected Site hazards or becomes exposed to the suspected Site <br /> hazards in an emergency incident <br /> ' 7.0 AIR MONITORING PROGRAM <br /> Monitoring shall be done initially to determine the presence of explosive conditions with a <br /> ' LEL reading of 10% or more by using a LEL meter Am organic vapor meter of <br /> photoionization detector will be used to detect the presence of volatile organics if levels of <br /> total hydrocarbons are detected (300 ppm - TLV for gasoline), the SSO will be informed and <br /> appropriate protective measures will be taken All monitoring equipment shall be calibrated <br /> prior to use <br /> ' 366-002 -6- <br />
