7-Eleven Store No. 19976 Page 2 of 2 <br /> 1399 North Main Street April 17, 2014 <br /> Manteca,.California 95336 <br /> • Following pressure grouting, enlarge the top three feet of the borehole at least one foot <br /> larger in diameter than the diameter of the auger originally utilized to install the well; cut <br /> off and remove the casing six inches above the bottom of the excavation before inserting <br /> the grout as described in the work plan; <br /> Notify the EHD at least two weeks prior to implementing fieldwork activities so that an <br /> inspector can schedule to be present for the well destructions; and <br /> - - -- -• Include-the signed and.dated manifests, of-wil and .gro_undwater waste removal when <br /> submitting the final well destruction report. <br /> The public participation period will end 20 April 20.14 and to date the END has received no <br /> comments or concerns from interested individuals and agencies regarding the pending closure <br /> of this site. Unless the EHD informs you otherwise, you may proceed with the well destructions <br /> if in compliance with the conditions noted above. <br /> Please submit an EHD Well and Boring Permit Application form along with a Master File Record <br /> form, encroachment permit issued by the City of Manteca and appropriate fees for each parcel. <br /> The permit fee for the destruction of each well is $125, along with a $375 Inspection Activity fee <br /> per parcel. A review of the site records indicate that ten (not eight as previously indicated by <br /> letter dated 24 December 2013) monitoring wells., three oxygen-injection sparge wells and five <br /> soil gas probes currently are installed. A fee of $3,000.00 (18 x $125 + $375 x 2) will be <br /> required for the well destruction permits.
