However, the <br /> following is still required to determine your eligibility for placement on the Priority List: <br /> A copy of the permit to own or operate the UST from San Joaquin County Environmental Health <br /> Department dated between January 1 , 1984 and January 1 , 1990 (pursuant to Chapter 6.7 of the Health <br /> and Safety Code) . If the tanks were not permitted prior to January 1, 1990, you will have to request <br /> the permit waiver. To request a waiver, complete the enclosed 'Permit Waiver Request" form and <br /> return with any additional information requested below. <br /> Your application states the tanks were removed on October 21 , 1999. Provide documentation from San <br /> Joaquin County Environmental Health Division that you obtained temporary closure prior to the <br /> December 22, 1998 upgrade compliance deadline. <br /> You are required to demonstrate financial responsibility. The financial responsibility forms that you <br /> provided are dated 1994. This is not acceptable. The forms must be current and you must list both <br /> mechanisms. Enclosed are forms and instructions.
