To request a <br /> waiver, complete the enclosed 'Permit Waiver Request" form and return with any additional <br /> information requested below. <br /> Your application states the tanks were removed on October 21 , 1999. Provide documentation <br /> from San Joaquin County Environmental Health Division that you applied for temporary closure <br /> prior to the December 22, 1998 upgrade compliance deadline. <br /> You are required to demonstrate financial responsibility.
