Installation of a single soil boring using mud rotary to approximately 150 feet bsg.The same boring is <br /> to be converted to a discretely screened(approximately 147.5 feet to 150 feet bsg) monitoring well <br /> MW-201 for the purpose of evaluating the vertical extent of the contamination. <br /> 2. The boring will be continuously logged from 110 feet bsg to final depth at 150 feet bsg. 5 soil samples <br /> are to be collected using 6.0 inch brass liners. <br /> 3. Groundwater samples will be collected using SimultProbe at depths of suitably coarse grained strata. <br /> COMMENTS AND CONDITIONS: <br /> 1. Continuously core the boring using acetate sleeve from 110 feet bsg to 150 feet bsg. Selection of the <br /> monitoring well screen section should be based on the field data and USCS descriptive log of this <br /> visual continuous core. <br /> 2. Must have additional slotted PVC sections on hand in the event that the targeted discrete geological <br /> strata is signif icantly thicker than the proposed 2.5 feet of MW-201's screen section. Generally,the <br /> screen section of a totally penetrated monitoring well below the shallow aquifer should span the entire <br /> thickness of the strata targeted for the monitoring well. <br /> 3. The work plan proposed using 0.020" slot size for the screen section of the monitoring well,but the <br /> attached diagram indicated 0.010" slot size is to be used for the screen section. PHS/EHD <br /> recommends maintaining a supply of different size slot PVC sections during the construction of the <br /> monitoring well. Selection of the PVC slot size should be based on the grain size of the strata where <br /> the well screen is set at. Communicate with PHS/EHD this decision before scheduling any work <br /> 4. The work plan did not specify the schedule size of the PVC to be used for the monitoring well. Due <br /> to the exothermic heat and pressure from the setting of 145 feet of cement/bentonine grout, <br /> PHS/EHD recommends consulting with the manufacturer on the specifications and tolerance of the <br /> PVC schedules.
