E1Dorado <br /> Stockton, CA 95206 <br /> San Joaquin County Public Health Services Environmental Health Division (PHS/EHD) <br /> approved a Work Plan prepared by Environeering, Inc. to investigate contamination at the <br /> above referenced site on February 8 , 1999. The work has not occurred. The work <br /> outlined in the above referenced work plan should be initiated by November 15, <br /> 1999. Remedial action needs to proceed as outlined in the approved work plan. If the <br /> work is not initiated by November 15 , 1999 the site will not be in compliance with <br /> PHS/EHD directives and reimbursement for corrective action costs from the State Water <br /> Resources Control Board Cleanup Fund may be jeopardized. <br /> Any changes in the above referenced work plan should be submitted to PHS/EHD for <br /> approval prior to work start date . A well permit application and $89.00 permit fee should <br /> be submitted to PHS/EHD as soon as possible. After the permit has been issued the work <br /> can be scheduled.
