I <br /> 05 July 2006 <br /> AGE-NC Project No. 97-0301 <br /> Page 2 of 2 <br /> PROCEDURES <br /> AGE proposes to properly destroy well MW-3 in accordance with Section 13 . 17 of the San Joaquin <br /> County Environmental Health Department Standards For Well Construction and Destruction in San <br /> Joaquin County. <br /> As required by EHD permitting, a health and safety plan will be prepared in accordance with <br /> Occupational Safety and Health Guidance Manual forHazardous Waste Site Activities (National Institute <br /> for Occupational Safety and Health Administration, U.S . Coast Guard and U. S . Environmental Protection <br /> Agency, 1985). Prior to drilling activities, the work area will be clearly marked and an utility clearance <br /> obtained through Underground Service Alert (USA). <br /> Prior to well abandonment activities at MW-3, abackhoe will be utilized to entirely expose the uppermost <br /> portion of of the 2-inch diameter PVC well casing from well MW-3 to determine its condition. The well <br /> will be sounded for total depth and checked for integrity; any obstruction found inside the well will be <br /> removed. <br /> Because the integrity of MW-3 was most likely compromised during the above referenced resurfacing <br /> activities, AGE is proposing to destroy the well by removing all material within the original borehole <br /> utilizing over-drill techniques. The abandonment of well MW-3 will be performed by Test America <br /> Drilling Corporation of Rancho Cordova utilizing a CME-75 mobile drill rig equipped with 10%4-inch <br /> hollow stem augers; well MW-3 will be over-drilled to a total depth of 50 feet bsg. The well casing from <br /> MW-3 will be removed from the over-drilled borehole and inspected by a representative of the SJCEHD. <br /> The above referenced well casing and drill cuttings will be containerized in properly labeled Department <br /> of Transportation (DOT)-approved model 17H 55 -gallon drums and stored on-site in an area lacking <br /> public access for future disposal. <br /> Total depth of the over-drilled boring for MW-3 will be measured. Thereafter, the excavated boring will <br /> be completely back filled with a cement grout mixture (Portland type II) in accordance with Section <br /> 13 . 12.5 . The grout mixture will consist of a ratio of six gallons water for every 94-pound sack of Portland <br /> type It cement.
