The location of <br /> the site is illustrated in Figure 1 ; site structures and monitoring well locations are depicted on Figure 2. <br /> Also enclosed with this work plan is a well destruction permit and the associated well destruction permit <br /> fee. <br /> INTRO DUCTION/BACKGROUND <br /> To date, a total of twelve ground water monitoring wells (MW- 1 through MW- 12) have been installed <br /> on- and off-site; ground water monitoring events have been performed at the site on a quarterly basis <br /> since December 1999 . However, it has come to AGE's attention that groundwater monitoring well MW-3 <br /> was recently buried by asphalt during resurfacing activities at the site by California Tank Lines (property <br /> owner); AGE was recently notified by California Tank Lines thatmonitoring well MW-3 was most likely <br /> damaged during the above referenced resurfacing. <br /> Ground water monitoring well MW-3 was previously installed in December 1999 to a depth of 50 feet <br /> below surface grade (bsg) utilizing 2-inch diameter PVC well casing and 0 .010-inch slotted screen <br /> installed from approximately 30-50 feet bsg. A report of findings documenting the installation of well <br /> MW-3 was previously submitted to the SJCEHD in Quarterly Report - December 1999, dated 02 <br /> February 2000 . <br /> In a letter dated 21 June 2006, the San Joaquin County Environmental Health Department (SJCEHD) <br /> directed that well MW-3 be located and exposed; the SJCEHD further directed that if the well was <br /> damaged, that the well must be immediately destroyed. Therefore, in preparation for locating and <br /> exposing this well, AGE has prepared the following well abandonment work plan in accordance with the <br /> above SJCEHD directive. <br /> � ; alvi � .
