A geologist contracted by ARCO may periodically
need to gain access to the monitoring well for data acquisition for approximately 1 year, or
as required by regulatory agencies. After data from the well are no longer needed, the well
will be properly destroyed by drilling it out with an 10-inch diameter auger and backfilling
the borehole with neat cement. The ground surface will then be restored to resemble the
surrounding ground material.
The undersigned has received a copy of and fully understands the Standard Conditions
governing the issuance of the revocable Permit. It is further understood that all changes
applicable to the preparation and issuance of the permit will be paid for by the undersigned.
Sincerely,
RESNA Industries
Original Signed lay
Jay R.
