You have listed <br /> the claimant name as Monty Warren, a partnership, with the Tax Identification Number 33-0042739. I <br /> believe the correct claimant name should be Enterprise Investments, in care of Monty Warren. The <br /> claimant should be the tank owner or operator,be named a responsible party, and be paying the cleanup <br /> costs. Please advise the Fund of the correct claimant name. <br /> Please correct page 3 of the application. In the site history section, claimant states that they purchased the <br /> property on May 25, 1998 from Whitehall Industrial Park. This is inconsistent with the listed history of <br /> property owners, UST owners,and UST operators of the site. Where do William and Mary Lana fit in as <br /> property owners? <br /> Claimant is required to have current financial responsibility documents on file. Enclosed is a Financial <br /> Responsibility Guide that explains the requirements. The Certification form is tabbed for your <br /> convenience and must be completed along with the applicable supporting documents. Please submit the <br /> original documents to the local regulatory agency and forward a copy to the Fund.
